1. Who I Am
Veteran Medical Opinions (“VMO,” “I,” “my”) is the solo private practice of David Protaziuk, MSN, APRN, FNP-C (NPI 1275200149, Illinois APRN License #277002570), operating from 7501 Lemont Rd, Suite 345D, Woodridge, IL 60517.
2. What I Provide
VMO provides independent medical opinions for veterans pursuing VA disability claims, including Nexus Letters, Disability Benefits Questionnaires (DBQs), Aggravation Opinions, TDIU Medical Opinions, and Aid & Attendance evaluations (VA Form 21-2680). All opinions are written and signed personally by David Protaziuk, MSN, APRN, FNP-C, based on review of the records you provide.

I make no representation or warranty regarding VA claim outcomes. The VA makes all rating, service connection, and benefit decisions independently. A medical opinion is one piece of evidence. No medical provider can guarantee a VA claim outcome, and you should be skeptical of any service that does.

Prices are flat-rate and listed at checkout:

- DBQ $249
- Nexus + DBQ Bundle $699 (saves $99 versus ordering separately)
- Aggravation Opinion $599
- TDIU Medical Opinion $649
- Aid & Attendance $649
Pricing is the same whether you order one condition or several. The Nexus + DBQ Bundle is the only combined discount offered. Payment is processed by Stripe at the time of order. Your card information is handled by Stripe and not stored on my servers.
Refunds: If after reviewing your records I cannot honestly write a favorable opinion, I will notify you and refund your payment in full before any document is prepared. Once a document has been written and delivered, a refund is not automatic, because the work has been performed and the document is yours to use regardless of the VA’s decision. However, if something has genuinely gone wrong, I review these situations personally on a case-by-case basis. Contact me directly and I’ll work with you to make it right, whether through a revision, a partial or full refund where warranted, or another solution.
Revisions: I offer free revisions on delivered opinions if anything needs clarification or adjustment. Email me directly to request revisions.
If the VA denies: If the VA issues any decision on your claim, email me with a copy of the decision letter and rating decision. I’ll personally review it and we’ll work out the best course of action together, including whether a rebuttal letter would help. I do not offer blanket free rebuttals, but I’ll give you an honest assessment of next steps at no cost.
7. Records You Provide
By submitting records to me, you represent that you have the legal right to share them and authorize me to use them solely for preparing your requested opinion. Records are handled per the Privacy Policy.
8. Limitations of Liability
To the maximum extent permitted by law, my total liability for any claim arising from the service is limited to the amount you paid for the specific service in question. I am not liable for indirect, incidental, consequential, special, or punitive damages.
9. Independent Contractor Status
VMO provides services as an independent medical provider. Nothing in these terms creates an employment, agency, partnership, or fiduciary relationship.
10. Governing Law
These terms are governed by the laws of the State of Illinois, without regard to conflict of laws principles. Any disputes shall be resolved in the state or federal courts located in DuPage County, Illinois.
11. Changes to These Terms
These terms may be updated periodically. Material changes will be communicated via email or prominent notice on the website. Continued use indicates acceptance.
