Not Affiliated with the VA
Veteran Medical Opinions is an independent private medical practice operated by David Protaziuk, MSN, APRN, FNP-C. VMO is NOT affiliated with, endorsed by, or sponsored by the U.S. Department of Veterans Affairs (VA), the Veterans Benefits Administration, the Veterans Health Administration, or any other government agency. The use of any military or veteran-related terms on this website is descriptive only and does not imply government affiliation.
Independent Medical Documentation Services Only
I provide independent medical opinions for use as evidence in VA disability claims. I do not represent veterans before the VA, file VA claims on behalf of veterans, provide legal advice or claim strategy, guarantee VA approval or ratings, or substitute for ongoing medical care, treatment, or therapy.
For Claim Filing Assistance
For free assistance with filing VA disability claims, consult an accredited Veterans Service Organization (VSO) such as DAV, VFW, AMVETS, or American Legion. You may also work with a VA-accredited attorney or claims agent. Find official accredited representatives at
va.gov/ogc/apps/accreditation.
No Guarantee of Claim Outcome
No medical provider can guarantee a VA claim outcome. The VA makes rating, service connection, and benefit determinations independently based on its own review of evidence. Medical opinions provided by VMO are one piece of evidence among many that the VA considers. Individual results vary based on factors specific to each veteran’s case.
Not Medical Treatment
The medical opinions provided are limited-purpose documentation prepared from records review. They are not a substitute for ongoing medical care, in-person examination, treatment, prescription medication, or therapy. If you require medical treatment, please consult your primary care provider, specialist, or VA healthcare team.
Not Legal Advice
Nothing on this website constitutes legal advice. References to VA regulations (e.g. 38 C.F.R. § 3.159, 38 C.F.R. § 3.310), case law (e.g. Nieves-Rodriguez v. Peake, Spicer v. McDonough), or VA processes are provided for informational purposes only. For legal advice regarding your VA claim, consult a VA-accredited attorney.
Testimonials and Reviews
Customer reviews and testimonials posted on this website or third-party platforms reflect individual experiences and do not guarantee similar results for other customers. Past outcomes are not indicative of future results.
Payment Covers Documentation, Not Outcomes
Payments made to VMO cover the time spent reviewing your records and preparing the requested medical documentation. Payment does not guarantee, and is not refundable based on, VA claim outcomes. See the Terms of Service for refund and revision policies.
Provider Credentials
Medical opinions are prepared and signed by David Protaziuk, MSN, APRN, FNP-C, a board-certified Family Nurse Practitioner with Full Practice Authority in Illinois (License #277002570, NPI #1275200149). David Protaziuk brings years of experience as a VA Compensation & Pension (C&P) examiner to every opinion prepared.
