There are two kinds. The first is a condition you had before service that service made worse beyond its natural progress. The second is a condition that is not service connected, made worse by one that is, such as a knee worn down by favoring a service-connected back.
My opinion reviews your records, sets where the condition stood before it got worse, and states whether service, or the service-connected condition, made it worse than it would have become on its own. That is the medical link the VA needs, and the one a lot of denied aggravation claims are missing.
