Federal Law Definition
The VA defines “competent medical evidence” as evidence from any qualified medical professional — NPs are explicitly included under 38 C.F.R. § 3.159(a)(1).
Cannot Be Rejected by Title
The VA cannot reject a medical opinion solely because it was written by an NP instead of an MD. The law is clear on this point.

Do you offer video visits or in-person exams for musculoskeletal conditions?
No. All opinions — including those for back, knee, shoulder, and other musculoskeletal conditions — are developed from existing medical records. This includes service treatment records, post-service treatment notes, imaging studies, physical therapy records, and prior C&P examination findings. Our role is to provide an independent medical opinion based on the documented evidence in your file. If a veteran needs new physical examination findings or range-of-motion testing, that should be obtained from a treating provider before requesting a nexus opinion. We will tell you upfront if your records lack the documentation needed to support a strong opinion.
What is the difference between a Nexus Letter and a DBQ?
A Nexus Letter is a medical opinion that explains the connection (or “nexus”) between a veteran’s current condition and military service. It is used to establish service connection. A Disability Benefits Questionnaire (DBQ) is a standardized VA form that documents the current severity of a condition for rating purposes. Many claims benefit from both: a nexus letter to establish service connection, and a DBQ to document severity for the rating decision.
